need to draft an exemption form?

we are registeres as homeschoolers this year and it is required we type up a letter stating the reasons we do not vax,( we are in florida so I beleive we claim religious reasons) anyhow is there a form out there i can just copy, i have searched but cant find one, can someone direct me in the right direction.

so i found this one has does this look?

We the parents of ________ are submitting a religious exemption in lieu of vaccination documents as per
Florida Statutes, Title XLVIII, Chapter 1003.22 which states The provisions of this section shall not apply if:

(a) The parent of the child objects in writing that the administration of immunizing agents conflicts with his or her religious tenets or practices;

Under the Constitution of the United States we are granted "freedom of religion." I know of no law that requires one to belong to an organized church or religious sect to practice his sacred beliefs. Please be advised and advise those concerned that I met the requirements under Title XLVIII, Chapter 1003.22.

I don't see that requirement in the law unless you are legally considered a private school http://www.immunizeflorida.org/community/law.htm and it appears to me that you do not qualify as a "private school" http://www.hslda.org/laws/analysis/Florida.pdf. I don't think you need an exemption at all.

For an exemption in Florida you need to go to the health department and get the DH Form 681, Religious Exemptions for Immunizations form signed by the nurse.

Florida mom here, and a former homeschooler!

You do NOT need to provide anything at all if you are a registered homeschooler.

If you enroll in a public or private school you would file a 681, as Emmeline mentioned.

Remember that vaccines are required for school entry. If you are not entering school, you are not under any sort of vaccine requirement whatsoever.

And even if you were enrolled in a virtual school (we've done that, too) you would not compose any sort of statement on your own, you would file a 681.

I'm not sure where you found the language in post #2, but that wouldn't be acceptable in any county in the state of FL.

and they require you send a letter as to why you do not vax i called and i was told i do not need a form just a letter claiming you do not vax and why.

I would love to hear your input regarding what i was told. thanks!

The school you listed is a private school and therefore requires an exemption. Go to your county health dept and get the exemption.
