You'll also get the occasional "Ion," but in general people recognize and totally get "Ian."

the name. FWIW, some people look at it with their eyes crossed, but it's really not THAT weird or different. Pretty much the same people who can't figure out Iain wouldn't know Ian, either. We have more trouble with the fact that my Iain has long, curly hair (girl?) than with the 4-letter spelling of his name.
