Thoughts on DD's rash...?

DD (almost 5 months) has had a rash/irritation for about a month, on and off. It started, IIRC, as an abrasive looking mark right where her prefold hit her belly (about an inch below her belly button). (She had just about outgrown that size.) I didn't worry too much, tried treating it with a random assortment of things... Mother Love Diaper Rash & Thrush, an occasional smear of Desitin (Mom bought it for me...). At some point, it got a little worse; some spots on her outer labia and inner thighs (not in the creases, not on her butt). I had changed to Charlie's soap a few weeks earlier, so I switched back, and rewashed all my dipes multiple times in hot hot hot water. I also abandoned my wipes solution (was a homemade, like Diaper Lotion Potion), and have just used water. I searched here, and then tried using breastmilk, coconut oil, and finally California Baby diaper cream. I was changing her what seemed like constantly, and leaving her diaper-free as much as possible, but it never seemed to clear fully. Finally, last week, I broke down and brought her to the doctor. Doctor said it doesn't look like yeast, and it does look just like heat/moisture irritation. Recommended the following: one week of Triple Paste, 3 days of hydrocortisone, disposable diapers/diaper free.

The next day, it looked to be clearing, and a few days later, it looked much better; could still see the original rash mark areas, but they were mostly her regular skin color. Over the weekend, we were traveling, and I was behind on laundry, so I brought 7th gen wipes. As we weren't home, she didn't have any real extended diaper-free time. Saturday looked good, Sunday night though, it looked redder again. She had been in a sposie longer than she should have been, maybe was the reason... maybe the wipes, maybe something else...?

So.... back home, I re-started the doctor's regimen without the sposies, unless I was going out, because I don't think they made a difference (it flared up again while wearing them after all). By yesterday, it was looking much better; the areas where she was rashy are still evident, but not red, just looking kind of dry/healing. Last night, as with every night we're at home, she slept diaperless. (I part-time EC, and catch her if she needs to go at night, and first thing in the morning.) This morning, the rash spots were redder/pinker again. The only things I can think of are: (1) I was kind of warm, maybe she was too, and could she be that sensitive? (2) Was it something I ate?

I've done endless reading here, and the only thing I've found that worked for someone else with a similar sounding story was Weleda diaper cream. But could another cream really make a difference? I've also been hoping this isn't related to something in my diet. (I EBF.) I though Sunday's flare-up could have been brought on by my eating a lot of a certain fruit last weekend, so I haven't had any since. Obviously that didn't do it. But would a food-related rash be only in a few spots on her labia, thighs, and belly? It totally looks like it's in spots that are chafed from her movements.

Thanks for reading if you made it this far. I'm exhausted! Any advice is appreciated.

Oh, one other thing, maybe helpful, maybe not... this morning, she was on her tummy diaperless, and peed... picked her up and she was really red all over her labia. So, does this mean she's super sensitive to heat/moisture?

It does kind of sound like a heat/moisture thing. My kids have all had food allergies and it doesn't sound like a reaction to me, but every kid reacts differently. I also wonder if she's sensitive to the wipes you're using. What fabric are they? I'd stay away from the sposie wipes for now, but I should also mention that my dd had rashes whenever we used cloth wipes (didn't matter if it was just water or some solution, she always had a rash) and we ended up using Costco brand wipes until she was out of diapers. I'd keep all creams off of it for now and see what happens.

Rashes are such a PITA.

My babe is in the same boat! I could've written this post!

We tried everything. Disposables instead of our normal cloth. Diaperless most of the day. Endless creams and salves. Wipes with homemade solution. Disposable wipes. Sitz baths. Ugh!

In the end what has worked for us is so stupidly simple. She is just incredibly sensitive to heat and moisture. We bought the California Baby brand of baby powder. It is corn starch based and has calendula, tea tree, and some other goodies in it. We put that on at every change and she has been rash free for almost a week straight!!!

I never would've thought to use powder until my mom suggested it. I had heard it wasn't good to use, but I guess that is in regards to the talcum based kinds.

Thanks for your reply (and for reading my long post!). I took her back to the ped today and saw a NP, who said it looks like yeast. (It looked very different at the office, much redder than at any other point.) So I'm using lotrimin on her now. NP said it could have been a heat/moisture rash that turned into yeast. Maybe that happened after having her in the disposables, specifically when I switched from the 7th generation brand to the Whole Foods store brand... I found those little pieces of gel on her skin after putting her in one this morning. Not using those anymore!

Interesting about water/cloth wipes irritating your DD. I thought about that. I was using mostly the same kind of wipes for a few months though before this all started. I stopped using the wipes solution... maybe I should go back to something though, as it had tea tree oil in it, which maybe would have helped prevent the yeast? Augh... who knows... You're right about rashes. This has been driving me nuts!!!

shapiro-strano, we posted at the same time, thank you for your reply too! I'm sorry you've been going through this as well. That's great about the powder though! I can imagine how wonderful a week with no rash must be! I will definitely try some of that as well. (I had briefly tried cornstarch a couple of weeks ago, but not consistently, as I wasn't sure I should be using it on her. But maybe I've had a little yeast in the mix too all along... )

I started using Lotrimin (clotrimazole) on her on Friday. It doesn't seem to be making a difference yet. For those of you who've used this stuff to treat yeast, how long has it taken to clear up?

I never used Lotrimin (always Monistat) but there were certainly times when we had yeast issues and the Monistat didn't touch it. Is she in cloth? If so, are you sure they are yeast free?

I use cloth (prefolds & pul covers, going to try some wool too), but she's hardly been in it for the past two weeks. I've had her either naked or in disposables with cream. I did turn my hot water heater all the way up and rewashed & dried all the cloth extra long though, so I doubt it's that.

My DD had a rash almost exactly like that when she was 6 months. It lasted for NINE WEEKS.

We saw three different peds, tried every diaper rash cream I could buy locally and at least one that I had to special order ($$!!), we switched soaps, wipe solutions, diaper styles, everything. Naked butt time, more baths, fewer baths, anti-fungal cream, prescription cream, fleece liners, no liners, everything. I even took her off solids to see if the bumps were acidity from her baby food.

You know what eventually worked for her? I looked at it, touched it, decided (in my oh so educated opinion) that it looked and felt like dry, irritated skin. I started slathering baby lotion on her at every change. 3 days later, healthy baby skin.

Every ped. told me it was from being wet and hot and that I needed to dry the area. Sometimes it would look better then get worse again. In the end, it just needed a little lotion.

I hope your solution comes quickly and simply!

Thanks! I actually had the same kind of "revelation" before reading your post, and decided to scale back a bit with the creams. I'm still using the antifungal, just in case, but I'm mostly just using olive & coconut oils on her with it, or a little California Baby cream if I'm out of the house. It's definitely looking better, although not 100%, but I think maybe she just has heat/moisture sensitive skin and needs a cream/barrier/stay-dry dipe during the summer. When I hold her under her arms or put her on the potty, she gets red marks that linger, so I guess it makes sense that she gets red where her dipes rub, especially when she's trying to crawl all the time.

I'll post updates if it doesn't get better... thanks for all the advice.
