Hi, my name is Carrie and I am a nursing student at DePaul University. I would really appreciate your help in completing my project about influences on parental immunization decisions. On the last page of your survey, there are a few questions that are not answerable as a simple true or false. You need to add another option to #2 and #5

Also question #23 is based on question #22 in regards to kids who got vaccine preventable diseases. Where they vaccinated? The only option is yes or no. In my case the answer is both. All my kids had cp- one was vaccinated, the other 3 were not.

I couldn't answer section 3 honestly. I agree that it needs an "other" category.
