REALLY high fever only at night?

My 13 month old has been running really high fevers in the middle of the night the last three days. Last night at two am. she was at 104.1!

When she wakes in the morning she either has no fever or a fever that goes down within half an hour without medicating. She starts to creep up towards 100 or so around lunchtime but I can get it back close to normal using motrin (tylenol wasn't working). It gets worse and worse as the day goes on and she is pretty hot by bedtime. Finally, she wakes up in the middle of the night, sobbing and sobbing, with readings like 104.1 or 103.5. And I can't get that fever down even with motrin and tylenol given in staggered doses.

LO has a slight stuffy nose but no other cold symptoms; though I have a horrible cold right now. And medicine keeps her pretty happy until around three or four in the afternoon when she gets clingy and upset. By bedtime she is clearly miserable and, if we keep her up late to observe her, DD is burning up and so lethargic she is only minimally responsive by nine pm. We brought her in to the doctor yesterday morning (we called the emergency line during the night and the doc. said we could wait until early morning to go in) but DD was completely fine by then.

What the heck is this? And is this a thing where I should be calling my ped. in a panic immediately or waiting until night to bring her in the ER so they have symptoms to observe or what?

I think this is a pretty normal cycle. I don't medicate during the day if they seem fine but I do at night if they can't rest. I would be concerned if a fever that high wouldn't come down with meds. The the dr do a complete exam? How about a strep test?
