Ice or heat?

I'm sorry to post so much about such a tasteless subject, but my hemorrhoids have taken over my life and I need some more advice.

I had one lanced last week but still have 3 rather large hemis that have not thrombosed at this point. I cannot get them inside for the life of me, so there they sit. I'm terrified what delivery will bring...my babies come out thundering fast and fairly large.

My midwife is going to fully support my poor bottom during the pushing phase. My question is what would be better? A hot pack or an ice pack? Midwife suggested ice, to prevent them from thrombosing from the heat, but I'm thinking I'll stretch better with heat? Either way, my last two pushing phases have lasted all of 4 minutes, so I'd like to be prepared in the moment. I guess we can just have both ready but I thought maybe there might be some experienced ladies here!

Thanks so much! I'm usually so eager for birth and PUMPED for it, but this is really weighing heavily on me and causing some anxiety.

Have you tried White Oak Bark Powder, directly on the hemis? I don't have experience myself, but a mw I know suggests this when other things fail.

Also, 'roids are related to varicose veins--are you taking any herbal supplements to support varicosities? This may help. Also, exercise and plenty of fiber--and NO straining at stool.

well heat would probably help your perineum and then ice right after baby is out. that is what has been done for me in the past. good luck! ouch.
