Vaccine Exemption in MD

I was wondering if someone could explain to me to me how to obtain vaccine exemption in MD? Do I have to apply for it at the Department of State Health? Do I need to hire a lawyer? Is it really hard to get?

At the bottom of page 1 is where you fill it out for religious exemption. All you do is sign that section and date it and turn it in to the school. It looks quite easy actually.
