planned VBAC may be compromised at 39wks due to unexpected complications, need info plz

i'm posting this for a friend who does not have internet. she is 39w4d today and is having some issues that may risk her out of HB. if anyone could point us toward some good info, i'd be most grateful.

here's a quick history:

#1 came at 26w, labor was 5 days on mag, pushing was 1 hr, baby was 1lb15oz, vag birth with many interventions but no pain meds

#2 came at 38w, labor was spontaneous, 3 hrs long, baby was 6lb12oz, vag birth, no pit, no pain meds

#3 came at 41w2d, labor was spontaneous, 3.5 hrs long, baby was 9lb5oz, vag birth attended by CNM in hospital, needed extra work pushing, but no pit, no pain meds

#4 came at 40w5d, labor was spontaneous, about 7 hrs long bc of conscious effort to slow it while awaiting a ride to the hospital, baby was about 7#, vag birth, no pit, no pain meds, but she arrived at 10cm and the LD nurses manually held baby in while waiting for OB.

#5 came at about 41wks. cx had started at about 37wks, then stopped. came back around due date, on and off for days. this is the one where everything fell apart. she went to the hospital too early in labor bc of family pressure (history of fast labor!) and the cascade of interventions finally caught up with her. she stayed in bed for 24 hrs getting IV antibx for GBS. reason for staying in bed was to keep labor at bay until 2nd dose to avoid fighting against baby going to NICU. bc of not moving around, her ds was poorly positioned. her OB was on a preplanned vacation and the back-up was the local high-risk guy, so despite her previous 4 vag births, he saw her as one big multiparous train wreck just waiting to happen on his watch. he coached her to start pushing at 8cm and she swelled so badly that she begged for a csec. the baby was around 7-7.5lbs

so...now she is only 20 mos pp from the csec and is due in a few days. mama is 33yo and has never taken good care of herself. she eats the SAD and has gone from petite to obese (she's only 4'11" and tiny-boned)throughout pg's #4 and 5, and now 6. the only way to get a VBAC around here is to HB, so she is planning on that, but her MW is starting to have some serious concerns.

the mama has had a very bad vaginal yeast infection throughout the entire pg. she's also had body acne, which the MW thinks may also be yeast-related. also, this is the 1st pg where mama has had swelling in her feet and ankles. its not pitting yet. her BP was very high, but she couldnt recall just how high. and her fundal height is 42, but really, she is very fat besides being pg, and i personally think the damage a csec does to your muscles and all the extra skin you end up with could mess with fundal height.

so now my friend is panicked that she will have such a large baby she wontbe able to push him out. i sort of talked her down from that one, bc even though she is so small framed, she already pushed out a 9lb5oz baby without help.

the midwife suggested garlic for the vaginal infection. i've come up with a llist of other things she can do for yeast, like coconut oil, gentian violet, peroxide rinses.

where could we go to look for BTDT info regarding the high BP? the MW has suggested that, overall, her diet needs to be changed like yesterday, so i've given her some good info on low glycemic foods. should she ask her MW about the brewer diet to stave off pre-e?

does this all sound like an attempt to risk her out of HB? around here, the MWs freak out over everything, even the CPM's. maybe the MW is just getting nervous about doing a VBAC?

also, if she hits 42 wks she will be transferred. i dont think she will risk an induction, so it'll be straight to ERC for her. but the alternative of herbing and castor-oiling out a baby at 39wks is scary. she is not sure of her dates bc she was nursing when she got pg. but she's never been sure of her dates and they've always come before the 42wk mark, and always spontaneously.

so any resources to whcih you coul dpoint us would be most appreciated, so she can take the info to her MW for advice. TY!!!

I'm not a birth professional - you might try posting there - but as I understand it, high BP is nothing to mess around with. Of course, it depends an awful lot on how high.

i would NOT mess with high blood pressure. a sudden increase in systolic pressure of 30mmHg or diastolic of 15mmHg is enough to worry about. with me, since i'm usually 110/70 or lower, 138/89 was dangerous for me even though, by hospital standards, it wasn't considered "high blood pressure". it happened to me with my first birth, and when my blood pressure reached that point, my kidneys decided to go on strike. i didn't pee for a few days. it could have progressed to life-threatening.

i'm not trying to scare you, just giving you a realistic view of how much high blood pressure can be a concern.

the other stuff, fundal height for example.. not so worried about that. if she's fat like me, fundal height won't be textbook anyway. my OB said that if the mom has a large pannus (that giant blob of fat below your belly button but above your pubis), the fundal height can be off by several cm.

The blood pressure and swelling would worry me the most.

I had pre-e and an induction with my first. When I mentioned that to my HB mws, they told me they had had luck with the Brewer diet (although I don't really believe in it, as I was eating massive protein with the first). They had one client with a high BP read who ate a rotisserie chicken and a dozen eggs in 24-48 hours, and her BP normalized. Totally anecdotal, but it might give her something to try to keep the BP down. If her BP stays high, she'll risk out of home birth for certain. High BP, even if it isn't pre-e, is a dangerous thing in a near-term woman.

As for the yeast infection, she should go on an anti-candida diet stat (compatible with Brewer - no grains or sugars, limited fruit. Lots of meat, eggs, veggies). If she truly has yeast on her body, she is at risk of getting thrush after the baby comes, and that is a very painful condition. (I suffered with it for 4 months and it was the diet that made the difference is keeping it away). If she weren't so close to delivery, I'd say she should do Monistat. It is the only thing that helped my yeast infection at 22 weeks.

Thank for all the info ladies. My bff had her baby at 4:30 today, via emergency c-section.

It's so sad that after 4 veginal births, a woman ends up with 2 csecs, but she went into labor spontaneously, no herbal induction, nipple stim, nothing. So her son chose his own birthday, his due date no less.

She labored for 7 hours at home and then her MW detected severe decels that just would not recover correctly. She was taken via ambulance to the hospy and had a c-birth under general anesthesia.

This is a real testament to how important it is to take proper care of yourself while pg. The baby is doing well, all things considered, and hopefully she'll stop at 6 kids.

I'm glad mama and baby are doing well, but sorry she had to have another c/s.
