Birth Certificate Forms & UP/UC

There is a section on the Oregon Birth Certificate Form titled PRENATAL.

It asks Principal payment for delivery - do I put "Self-Pay" or "Other" and write in No Charge?

It also asks for First and last prenatal visits and total number of visits. There is also a box to check for No Prenatal Care. I have a feeling I'm supposed to check that box, but that implies that I had no prenatal care, when I did...I just did my own. I kept track of everything I did - urinalysis, BP, FHR, etc... Do I count those as "visits?" It doesn't say "medical professional prenatal care."

but in there somewhere is a list by state of the forms and directions for filling out. I easily found it the other day, but now that I'm looking for it I can not find it! Go figure! Maybe somone else can provide the link? I have the forms saved to my computer but I'm not sure how to attach documents on this forum?????

looking at the forms I would check other and write in unassisted childbirth. and check the "no prenatal care" They're looking for supervised information a doc, mw, fp anyone outside the home that can say I gave so and so prenatal care. at least that is my interpertation of it. BUT I say put whatever you feel is right for you. It doesn't ask for any pcp info so they may not really care.. I know my state is much stricter and wants all the "official" info not just what I have to say about it (hence the OB backup i have) I don't want any issues getting a birth certificate.

we have a lady that all she does is homebirth bc's so i called her and she answered all my questions

For my children's BC's I put "self care" where appropriate. Self care is very different than no care IMO. (Yes, I'm paranoid about some well meaning busybody calling CPS on me so I at the time, I felt the need to indicate that we had care, just not one that fit into one of the boxes.)

I filled in "not applicable" where that was appropriate too.

My experience both times tells me that the folks taking the forms from you will ask for verbal clarification an anything unusual anyway. Let's hope the person you deal with will not be completely unfamiliar with the concept of home birth, like the person was for my first birth. The poor woman asked me 5 or 6 times to explain where DD was born and "at home" just did not register with her. Ha ha ha.
