New and Question re: Gas Pains and Waking to Nurse

Hoping this is the right place for this! I am a new mom with a three week old who seems to be having some bad gas pains after nursing, which happens frequently these days (usually every 1-2 hours during the day). By way of background, I occasionally need to wake her after 4 hours overnight to nurse, as instructed by my midwife. My supply is good (if anything a bit too much at times?) and she is gaining weight and pooping a lot.

Today, she had the worst day yet for gas, as it seems to have affected her sleep. She slept maybe an hour altogether today with some quiet alert periods, but she would not let me put her down - and this evening she became simply inconsolable. She gets red in the face and then wails sharply when I move her slightly - wondering if it's gas, because sometimes I can get a few burps out of her by sitting her upright. She has been eating all day, which I think must contribute to the bad gas because she tends to gulp at the breast - esp. if she has been crying already.

We finally swaddled her and she fell asleep. My question is whether it's better to let her get a good sleep tonight, no matter how long that takes, since she ate a lot today but barely slept? Or should I stick by the four-hour rule and wake her to nurse? I don't have a pump yet so that's not an option for me.

I realize that it's already late and I will probably just need to go to bed without an answer and make a game time decision ... but I appreciate any advice! Thank you.

Let me see what I can address. First, you mentioned some burps came up when you sat her upright-- are you burping her thoroughly after each feeding? My Cecilia often needed to burp 3 or 4 times after a nursing session, so stopping after one burp might not be enough.

Excessive gassiness is, sadly, part and parcel of lots of young babies. Cece was that way too, and thankfully has outgrown much of it now (4.5 months). Here's what I would do:

When it's almost nap time, take her to the changing table, and get her situated there. Do knees to belly with a little bit of firm pressure repeatedly, until she starts tooting. Keep going until no more come out. Then do your diaper change and swaddle and whatever else you do for naps.

After nap, you can try massage: use your fingers to press somewhat firmly in clockwise circles around her belly button. You can use a little oil or lotion if you want to make it easier.

The more burps and help you can give her for passing the gas, the more comfortable she'll be for sleeping. And hopefully she'll be like Cecilia and outgrow most of it!

I can't offer you any help for the night feedings as Cece always woke up every 2-3 hours to nurse until she was 2 months old.

3 weeks = growth spurt, so expect lots of nursing! she may also nurse all night.

if a baby is back to birth weight and gaining at least 1 oz (~30 g) a day, and isn't jaundiced, then I would let her sleep. i would wake to feed after 5-6 hours though for my comfort!

Thanks! So last night I decided to set my alarm for 4:00 a.m. since she was last fed around 10:00 p.m., with the fact in mind that she had lots of little cluster feeds right before falling asleep and throughout the day. She ended up waking me anyway at 3:00. This time I tried nursing on and off with frequent burping in between - and yes, this time allowing for a longer burp period with 2-3 burps at a time before resuming feeding. I was able to get her to sleep in our bed until around 6:30 (moved from crib where she had fallen asleep at 10), then woke up and repeated the process. This time she was fussier and less inclined to go back to sleep, we swaddled her and now she is sleeping again. I will also try the tummy massage next time (I do rub my whole hand on her tummy and "pump" her legs but I will try the circular massage with fingers.)

I still have not gotten a sense of her usual nap and feeding times, and every day seems a little bit different. I did start keeping a journal so maybe if I go back over the past several days a pattern will start to emerge.

Don't worry, it's totally normal to have no discernable pattern this early. I would just assume that every fuss is for eating and sleeping, if it's not for pooping. I don't think that Cecilia had a distinct pattern until well over 2 months, when she slipped into the nap/sleep two hours after waking up pattern that she is still holding now at 4.5 months.
