who makes the TD shot?

I have a 33 mos old daughter who has never been vaxed but I have decided that I am interested in getting her the tetanus shot and since she to young to get just the T am wondering about getting her the TD.
((She is a at my parents all the time and they have a small property with horses and lots of nails/wood(think splinters) and my mom always has my little girl working in the garden/working the property with her and cleaning up after the horses....sometimes shoeless or even half naked(!).))
So I am going into her Dr.'s in about a week and a half and I need to know what to ask for. I'm pretty sure they won't have anything other than the most standard DTaP because this is basically a state run gig servicing the un-insured, under insured and medicaiders.
So it would have to ordered and what do I do, pick it up myself from the pharmacy, does the Dr. order it?

DT is made by Sanofi.

I don't live in the US, so I can't help with info about how to get the vaccine. Where I live it is available at the clinic, as well as the Pentacel (DTaP, IPV, HiB combination vaccine).
