Honestly, I wouldn't block feed at this point. Block feeding is designed to regulate your milk supply in the case of oversupply. I do think it's important that she get the hind milk, which block feeding can help accomplish, but rather than block feeding, try expressing a few tbsp of fore milk off before latching her on and do the compressions at the end of the feed when she starts slowing down her sucking.

I haven't read the responses, but basically, I had your exact situation: sleepy baby who didn't gain weight in the first two weeks, ped (and LC) recommending supplementation.

In my opinion, looking back, it was the sleepiness that did me in. That child simply did not nurse enough in those first two weeks to get my supply up and running, and by the time he stopped falling asleep at every nursing session, I was making only enough for him to maintain his weight, but not enough to gain. Every time I stopped supplementing him, to see if his newly wakened state would produce more milk, he just didn't gain weight.

However. That was my first child. You are an experienced mother with a successfull bf history, so you have a huge advantage on me. Here's what I would do. Give her the supplement. I don't care what anyone says, a 3 oz weight gain over two weeks is not enough, not good.

I would do the formula for 48 hours to rule out other issues, like the ped suggested. And in the meantime, pump like a madwoman. Pump like crazy to get your supply up. (I understand what a huge pita this is, especially when sleep deprived). Because I honestly think that a kid who falls asleep all the time - it would take me literally an hour to get my babe to "nurse" the equivalent of 10 minutes, what with trying to wake him up - is going to mess with your supply in those early days.

I don't think you are doing anything wrong. You just got handed a sleepy nurser. And that is screwing with your supply AND your baby's intake.

In my situation, I tried for 4 months with the supplementation and the weighing and the taking him off and on formula and the anxiety about his weight gain, and I finally said, to hell with this. I wasn't enjoying being a mother, I hated bf, and I was so consumed with whether he was starving to death everytime he cried. I put him on formula for his nutritional needs and continued to nurse him whenever he wanted (which turned out to be 2.5 years), figuring that whatever milk he got from me was a good thing.

I haven't supplemented much because after 40 mins of nursing she's done 90% of the time. And I am wondering if she's actually eating too much now, or if there is something else going on...

Since I started nursing her 20 mins each side she's spitting up a lot more. Like just now I nursed her 20 mins on the left side, used breast compressions, heard some GREAT swallowing, but then she spit up 3 times, it's white and curdy not vomit, and one time it was about as big as a grape. DS never spit up, ever, he never even burped lol, so I don't know how much spit up is normal for a BF baby, or could we be dealing with reflux. She's always spit up a little, now that she's getting more food there is mor spit up.... I think she's gained some weight though, but that could be wishful thinking....

all that spit up and the curds sounds like DD1. do you drink cows mik? if so, i would cut it out. she doesnt sound like she needs supplemented either. formula can upset their gut flora. i would let her nurse on one side till she is done. change her diaper, burp her, and offer the other side. if you fed the first kid fine then you know you make enough. my DD1 didnt gain and is still small, but she is just right for her.

I'd stop the formula and find a breastfeeding friendly pediatrician. By output alone, it's clear this baby is getting enough breastmilk- that doesn't mean she shouldn't be followed for weight gain or brought in for check ups, but it should be to someone who isn't so quick to supplement. Try your state in the finding your tribe forums. And yes, set the alarm to nurse at night so she doesn't go more than 4 hours- I know it's hard, I had to do that, too.

do lots of burping. if you are using the shield, she's likely getting a ton of air. i also supplemented under duress and i didn't really need to. i'll bet your dr is using formula fed baby comparisons/weight gain. they wanted my PREMATURE BABY to gain 2 oz/day. just keep on doing what you're doing and watching her diaper (pee) out put.

I think boobs4milk is right - burp way more often than you are. I know that is a pain with the latch issues but it really sounds like you have plenty of milk but that she is spitting up enough to reduce her weight gain. I took care of a premie in the NICU that spit up her feedings. She weighed 2 pounds and was perfect except that she only gained weight on my 3 day weekends. I was the only nurse that did not have her spitting up her whole meal. BUT it was because I burped her every couple of ccs. It took an incredible amount of time to feed, burp, feed, burp, feed, burp but it was what her body needed. The shield will make your baby take in more air so she may just need to be able to let that out more often. Also, express the foremilk a bit before you latch her on. This will get her some of the more high cal stuff before she goes to sleep.
This will get better!!!!

thanks everyone. She got another 2 ounces of formula yesterday after a not so great nursing session, she jsut didn't want my left side, she did great on the right but just wouldn't suck at the left. but then she promptly spit up a ton of the formula lol,
I think you are right about the burping, usually I only burp her between sides or if she acts like she needs it. I will start doing it more. We go back to the ped in a couple hours, I am going to get in one more good feed before we go.... and she woke herself up after 2.5 hours last night so I didn't have to set an alarm thank goodness because I overslept to bring DS to school anyway... thankfully we weren't late, but I just can't get enough sleep

7lb3oz, up 3 ounces from Wednesday, only 6.5 ounces of formula in the 48 hours... the ped was "surprised" that she gained so much without the formula... hmm maybe it's because nursing is enough????? Honestly I think she just wasn't nursing "good enough" and I took for granted that she would be, since DS nursed just fine, and when I got vigilent about watching her more closely forcing her to stay awake to nurse, and watching the clock more she did better it seems to have made her get into the habit, she's started staying awake a lot better while she's nursing. I am so happy she's gained and the dr obviously isn't concerned anymore since he doesn't want to see her for 6 more weeks

great update! I also wanted to say-- and you probably already know this-- that my ds used to fall asleep at teh breast pretty early in the early days. So even if he was on the boob and sucking for 20 minutes, it would be more shallow comfort sucks, not the deep nutritive sucking.

We used to undress him, or jiggle him midway through nursing to get him started back up again.

undressing isn't enough for her lol I have to change her diaper, rub her cheeks or tickle her sides.... but she's getting much much better about it.
