Medical release forms or notes...

I need to send my teenager to a doctor by herself. This is a new doctor, and I've never even met them.

But, they said to send her in with a signed release form saying I give my permission for them to treat her.

I can't find one online. Do you know of any? Or is a hand written note OK?

I would call them back and ask if they have a specific one they want signed, and can they fax it to you, or if a hand written note is enough.

They should send you one in an information packet. This is what they always did for my mother when I was a teenager.
