Help with Fasting Blood Sugar? - Diabetes Type 2

I was hoping someone might have some advice for me.

I was recently diagnosed with Type 2, though I've had PCOS / Insulin Resistance since I was a teenager. So, I knew it was coming.

I also recently found out we're expecting, and am 9 weeks pregnant.

I'm on 2.5mg of Glyburide twice a day, and a meal plan, but I'm having trouble getting my numbers where they should be. My doctor happy with my numbers, but I know from past experiences, they should be lower. With both my past pregnancies, they were supposed to be under 150 two hours after a meal, and under 100 for fasting.

My two hour numbers are consistently under 160, but my fasting numbers are anywhere from 117-143...

I eat six times a day, and my final snack at night is 2 starches & 1/2 milk. I haven't seen any pattern either. I usually have cereal & milk, and they're still all over the place.

Does anyone have any advice on what I might be able to do to get that fasting number down?

Thanks!

Hmmm. I had GD with dd2 and I seem to remember the 2-hour postprandial number was supposed to be under 140? 160 seems kinda high. What are you eating protein wise? Are you counting the milk in your cereal as a protein?

ETA I meant up to 160 pp seems kind of high, sorry.

Also wanted to add there is a Gestational Diabetes support thread here in the I'm Pregnant forum, the ladies there will probably have some ideas for you too.

Yes, since my last pregnancy they've lowered it from 150 to 140.

I know my numbers are higher than recommended for GD, though my OB says they look good - I still want them lower.

I have a very specific meal plan from a nutritionist and I have protein with every meal & snack, except the before bedtime snack which is just starch & milk - milk is not counted as a protein with any other meal/snack.

We just adjusted my meds again on Tuesday, so we'll see. If this doesn't work, then I'll have to meet up with a nutritionist again and see what can be done. I have hypoglycemic episodes on the higher dose, but the lower dose doesn't seem to be enough.

Just frustrating! In a month, we've changed the meds three times.

I had a hard time getting my numbers in check too. In my case, I couldn't have milk at all because it consistently gave me higher numbers. It took awhile of playing with different protein/carb combos to figure it all out. My bed time snack was usually a handful of crackers with a dollop of greek yogurt, I had to have the protein to keep my fasting numbers down. Definitely talk to your nutritionist about tweaking your meal plan if the meds don't help.

I was also diagnosed as a t2 diabetic early in my last pregnancy. I couldn't get my numbers under control with pills so I switched to insulin, which wasn't a big deal with me as would offer me more direct control. My Dr. was going to have me go off the pills a few weeks before delivery anyway and switch me to insulin for the duration of our breastfeeding.

I don't say it was super easy, because I did battle hard in the end when you fight the placentas hormones that counter the insulin, but it wasn't too bad and I did feel better than I had before the change.

Greenluv - I was wondering how the insulin would be, but I don't do well with needles at all. I gave myself one b12 injection and couldn't do it again. But I know I'll be doing it at the end of pregnancy...

At our last appointment, he said he was going to be mean and tightened the reigns. He upped my meds, and wants my fasting under 90, and 2 hour under 120. No problem with the 2 hour - it's usually 100-105 - and the fasting was down to 99 today. I'm just happy it's under 110 which is consistently now - usually 103.

But I'm so hungry all the time

I thought of something yesterday that might be affecting it. I take my thyroid medication at night, and since that is related to hormones - the surge of thyroid, may be upping my blood glucose a bit. Otherwise, it might be getting too low at night causing a glucose dump... I go to bed hungry, wake up in the middle of the night hungry, and wake up hungry...like stomach growling, nauseous hunger. Still my number isn't hypoglycemic by any means. I need to test when I wake up at night and see if this could be the case.
