ISO supply-increasing ideas

My friend has a 9-week-old and is struggling terribly with a low supply. She has only been able to pump around 5 oz in a 10 hour work day - she works 2-3 days a week. She is not at all interested in supplementing with formula but is worried that her l.o. isn't getting enough - isn't gaining enough weight (she was well over 8 lbs at birth and is now not quite to 11, I think) - she just seems like she isn't eating enough. Her latch is good and she nurses frequently now. At first the ped (who is no longer their ped) had them on a schedule, nurse something like every 3 hours and no more even when the baby demands it. The ped told dad to take the baby out of the room away from mama because the baby would demand if she smelled mama... isn't that the whole point? Anyway, they followed that for about a week, til they figured out that the ped was nuts, but now she's afraid that her supply didn't get off to a good start.
She has been a very stressed mama, too. A storm took away their electricity for a week, then their water supply disappeared and she was going between her parents and in-laws for a while. I know that the usual belief is that whatever mama makes is what the baby needs, but I think she has some valid worries that her supply is too low. I always had an over-supply whether I wanted it or not, and I've never had to think about how to increase. Can you please share what has worked for you?

a few more things -
AF returned at about 7 weeks pp.
Baby poo was not plentiful - at first, she'd go 4 days with no poo. Now she's every other day.
No IVs during birth.

First of all, how much you can pump doesn't indicate her supply, but if she is only getting 5 oz, I can definitely understand that she wants to get more. First though, unless baby isn't growing well, acting hungry all the time, not pooping enough, etc. her supply is likely fine. If she is still worried, she should call or go see a LC and get whatever she needs checked out to make sure her worries are needless.

How often is she pumping during that 10 hour work day? I would recommend 3 times at least. Breast compressions help me a lot when pumping to get more out, have her look them up online, I think Jack Newman has a good explanation and video on his site. Does she have a good quality new pump or a hospital rental? Has she messed with the pump settings to try and get more. Some people find light distraction to help more while pumping, others focusing on a picture/blanket/clothing item etc. of baby works better. Relaxing while pumping and getting enough rest are the big ones for me. Power pumping also helps me (sessions of 10 min on, 5 min off, 10 min on, etc. for as long as I can) to increase supply too. The big thing to remember to increase supply is that you have to increase demand by getting more milk out as often as possible.

Beyond that, there are herbs and whatnot you can take, but I would try all the non-herb stuff first and see. And make sure to give things a few days, it takes a bit for supply to respond to stimulation.

Has she tried eating oatmeal? That always gets my supply going! I find that plain, old-fashioned oats work better than instant. Oh, and she should make sure that she is drinking LOTS of water.

Beyond that she should just nurse, nurse, nurse. Pumping after nursing will also help stimulate her supply, even if she doesn't get a lot out with the pump.

it sounds like a few days of enough water and rest and he supply would pop back to whatever her baby needs. Just since you mention stress and multiple residences and water problems. that's a lot of disruption in addition to the work and fundky dr's schedule.
she probably doesn't need herbs, and baby proably gets much more than the pump does. if it was my friend, I'd try to get her lots of quiet time to nurse and plenty of water.

Motherlove makes a great herbal blend called More Milk (and More Milk Plus after a certain age) that really work well to boost supply. Other than that, hospital grade pumps are best for building supply but for plenty of supply/older babes the PIS variety usually work fine. Also, it's essential that the cones fit well (according to boob/nipple size).

I'm sorry to hear she got such bad advice from a doctor, but unfortunately it's pretty common. I'm sure she can get it back up with effort, perhaps encourage her to take a nursing vacation and try these other tips from Kellymom. http://www.kellymom.com/bf/supply/low-supply.html
