I am working with a mama through La Leche League (she sometimes posts here, not sure if she'll see this and comment or not). Her baby had Pyloric Stenosis at birth, repaired at 4 weeks old (I believe?). He also has had a tongue clip twice (once by a ped, once by a dentist)
He is now 3 months old. Developmentally he is doing well, but he is NOT gaining weight. He's growing in length and head circumference (I'm not sure what his curve is, but he is gaining in both areas) but his weight is very low.
Help me brainstorm here!
--He appears to be transferring milk (weigh-feed-weigh shows up to 3oz).
--Mom appears to be making plenty of milk (she pumps well, and is tandeming and her older nursling reports good output)
--Baby is still spitting up a lot (is on Zantac, hopefully will be moved to Prevacid soon).
--There is some concern of a high palate (which would go along with the tongue tie) but again baby does appear to be transferring milk despite that.
--Baby is peeing well, pooping infrequently though (and it's a strange smell and consistency).
Is anyone aware of other medical conditions that can accompany Pyloric Stenosis? The repair was done by a General Surgeon, not a GI, baby has not had any other workups (no upper GI, no swallow study, no barium enema, no cardiac workup, no kidney workup, nothing)
Ped, of course, has recommended formula, but this does not appear to be a breastfeeding or breastmilk issue, at least not solely.
What else should be considered here? Mom is working on increasing hind milk intake, is keeping a log of when he feeds, how long, which side, when she pumps, how much output, how much fore/hind after the milk separates, when baby pees, poops, sleeps, etc. Mom is working closely with Lactation Consultants and several LLL leaders. Mom has successfully breastfed her other children for many years, definitely knows what she is doing!
