What to do after a homebirth in MO?

Alot of the information on homebirthing, and what to do afterwards, is somewhat unavailable and hard to find, since homebirths in MO have just been re-legalised within the last year or two. I know you can send off for the birth cert, but those of you who've done this recently, what did you need? affadavit? signed piece of paper? i'm also wanting to do this unassisted, so do I need a midwife's signature? I'm pretty sure I don't, but I'd like to make sure.

Also, our insurance...sucks, so we'd like to have the kids on medicaid, just for checkups. do we need SSN and Birth cert for that?

is there anything else i need to know? any snag they'll try to catch me on?

I can't speak to MO specifically, but I had a HB in NC attended by a CPM who is "illegal" in that state, like all CPMs. I went down to the Vital Records office, submitted the form, and had no problems. I didn't mention the MW--when they asked who delivered the baby, I said "we did" (dh and I), and they didn't blink.

You may need to supply proof of pregnancy (an ultrasound, a doctor's preg test, etc), and might need to bring the baby as "proof of live birth", but you can probably look that info up. You may also need a "witness" (your dh will do). But I brought all that stuff along and I don't think they ever asked for it.

You can apply for a SS card when you submit the birth certificate registration form. Insurance wasn't a problem--just tell them you're waiting for the number, and fax them a copy of the receipt they give you when you submit the birth registration.

I had my first HB in NY. The midwife did all the paperwork for the birth certificate and social security number. She also gave me a certificate. It looked like the pre-printed certificates you get in school where the teacher fills in some blanks. But it was pre-printed to say that the baby was born at home and some of the same details as would be on a birth certificate, and then she put the baby's footprints on it.

Anyway, when I called the insurance co, they said if I faxed them the foot prints, they would take that as proof of live birth, so I wouldn't have to wait for the other stuff to arrive. Even if you have to wait (we just moved a month ago and still don't have the proof of insurance, even though we are covered) the doctors can often hold your paperwork and bill to your insurance when you get your info.

because of the UC I had to apply to get the form from Jefferson City then have 2 friends notarize it with me and send it back. I got my BC and SS card a few weeks later. wasn't a big deal at all. didnt even require proof of PG (didn't have it anyway as I did UP too)

call the state office of vital records and ask for Kim (she handles home births). She'll tell you how to request the application. (You have to send a letter and $15 just to get the application). 573-751-6378 is the number. The letter you send has to include very specific information. And the check has to be written a certain way. when I filled out the BC I called and asked any question to make sure that I was filling it out right.
