confused about ballet enrollment? Should I just give a date?

i just enrolled my dds in ballet and i was required to sign a ton of release forms, which i figured we would have to, (boy times have changed since i took ballet) and then i was required to write down my health ins co with policy numbers, i was required to give pediatrician name and number, and then at the bottom of the form i was required to put my dds last dtap and some other vaccine dates.... why?

so i left that blank along with their ped's phone number and the ins policy id numbers and stated i didnt know that info off the top of my head.... and the instructor was very firm about me remembering to bring this info to the next class... any opinions what i should do as far as giving them a date of my dds last vaccines, do i write we are exempt from them and hope they let my children continue class or do i just fib and put a date down.... what would you do in this case? I am learning toward just gving a false date, why bother to get all invovled in a debate and possible dissmall of the class>

First of all, I would not be giving a dance instructor my insurance policy numbers! That is just weird. Why would they want those? Also pediatrician numbers?

I would want tof know why they need that information. If something were to happen to your child in class, they would need to call YOU, not the pediatrician for any non-emergency. For any emergency, I can see them having you sign a release letting them taker her to the ER or call an ambulance or something.

As to your question however, that is a totally personal choice. I don't know how you feel about lying about these kinds of things. I would be inclined to tell them I have a religious exemption and leave it at that. If they tell me my child cannot enroll, I would point out that that is religious discrimination and see how that goes over. At the end of the day, it's just a ballet class. Not worth it IMO.

My DS plays for an MLS (major league soccer) youth team and I also had to fill out an extensive health form. The club has a policy that every child had to be up to date with all vaccines recommended by the APA. My DS is completely unvaccinated, and obviously we wouldn't vaccinate just for him to play at this club. So we submitted the school vaccination form (which allows for all exemptions) with the religious exemption signed, along with a signed copy of this letter:

To whom it may concern;



(We / I) {First and Last name(s)}, as the {(parent (s) / guardian(s)} of ______________________(name of newborn child) are exercising (our/my) rights under the US Constitution, Army Regulation 40-562, BUMEDINST 6230.15A, AF JI 48-110, CG COMDTINST M6230.4F , section 3.2, para4, and Army Regulation 608-10, section 4-6, para 2, to receive Religious Exemption from Vaccination, due to our genuine and sincere religious beliefs which are contrary to the practices herein required.


The U.S. Supreme Court held in Frazee V. Illinois Dept. of Security, 489 U.S. 829, that a religious belief is subject to protection even though no religious group espouses such beliefs or the fact that the religious group to which the individual professes to belong may not advocate or require such belief. This ruling is also reflected in Title VII of the Civil Rights Act of 1964 as amended Nov. 1, 1980; Part 1605.1-Guidelines on Discrimination Because of Religion.

I cannot imagine why you would need to provide all of that just to take a dance class. Unless it's some sort of mother's day out program or something that falls under daycare licensing. On field trip & scouting forms I generally just write "current" or something, I never put a date. I also wouldn't give out my policy numbers.
