mamas of intact baby boys...need some help please?

DS is 20 months old, intact and CDed.

A few weeks ago, he had a small "dab" of white stuff in his diaper in the front. DH and I thought maybe a piece of pasta got smushed in there since he was eating it and making a mess. Figured it just fell in there and got smushed. We noticed no issues on him physically (no redness, etc).

Well, last night around 2am, he seemed uncomfortable in his regular nighttime diaper (crying out, pulling at it and waking a LOT) so I took him out of it and put him in his regular Muttaqin Baby with an added insert and threw some wool shorties on over. He slept the rest of the night much better.

This morning, I took it off and there was that same "dab" of white stuff (a little more this time) but part of his foreskin is a bit red and swollen at the tip. He doesn't seem bothered by it (I didn't obviously pull anything back, but I did examine it closely) as he was smiles while I examined him.

This is our first boy (4 year old DD) and I had no brothers...so I have NO idea what is normal, not normal or concerning.

Any advice would be welcome...especially since our pedi (while fantastic about not vaxing) has no clue about foreskin care.

My ds has had a red swollen tip a couple of times and the advice from mama's here was a baking soda bath that cleared it up. I've never noticed any white stuff though.

Part of the separation process sometimes involves smegma pearls. They develop under the foreskin and work their way out. As long as there is no odor and no pain involved I wouldn't worry much about it.

Thanks mamas. Wouldn't I have noticed the pearl if it was there last night though? Prior to working its way out, I mean.

And isn't 20 months really early for this process to start?

Separation can start . It usually begins in toddlerhood, but anytime between birth and puberty id within normal range

There is no specific timeframe. It can start super early (as in infancy) or super late (early teen years). I have one boy that started before 3 and one that is still not retractile at 6.5

and smegma pearls can be somewhat firm or squishy so its possible it wasnt seen last night.

like Fyrestorm said, if there is not real pain (there can be some discomfort for a day or two with separation) or foul odor, there should not be an issue

Could be yeast infection

If the white looks cheesey, like ricotta, could be yeast. Just dab a little lotramine on once a day and it should clear up. My oldest got yeasty on occassion. And that took care of it.

The only time DS1 got an infection, you could tell straight away what was wrong. It went from looking a little red to crying when he peed and pus on his nappy. We got AB for that. Otherwise both boys have had the odd red foreskin but always seems to clear up OK.

Also, on the retraction front - DS1 has been able to retract his for a while. Since he was about 4.5yo?
