School Nursed called about exemption form

I have two children with two different last names. They are in the same school district but two different schools. We have also used the states on-line school services for all 3 children and the local elementary school three times.

I go down to the Department of Health, pick up the appropriate form and fill out, sign, turn in. It is accepted and we move on.

This nurse was funny. She insisted it should be a different color. The last one she saw was. I told her were I got my form from County Health, while looking up the state information on-line. I let her know I was informed this was the correct form by the state and county health. She insisted it could not be. I asked her what the proper form number was. She told me I had to get it from my religious leader. I pressured her, how am I to know if we are doing it right if she does not know what form number it should be. I then quoted her from the state's link.

She said she would get back to me. About 15 minutes later she calls back and says I needed for 11a. I turned in 11. (I smell something fishy). She will get a copy for me and send it home through my child (Things smell fisher). She is persistent that my minister fills it out. She is going to be disappointed when she sees she has the same form I turned in and that ministers are no longer required to fill them out.

Explain to me, how you know that you have the wrong form if you don't know which form number you are suppose to have?

She also try to tell me I had to go get it. But how do I know if I have the right form if she does not know the right form number?

Tell her to put all of her objections in writing. No more oral communiques.

I know it is form imm.p.11a That is what I turned in.

She just did not know what she was looking at.

How can she know this was not the right form, if she did not have the right form number?

You wrote that you turned in the form 11 and that she got off the phone with someone and said you needed the 11A?

If you are confident you have the right form I would say that the form you turned in is the one required by law, and if there is a problem the DOH can contact you. Then drop a copy of the law with the form # highlighted on her desk. If she continues to harass you I'd speak to the principle.

even without knowing the correct form number she could recognize that the categories are different than the ones she's accepted previously. and, as emmeline pointed out, if 11 and 11a are the same thing then you should be covered.

I didn't ask for my card back, but what she sent home seem to be exactly like what I signed.

I got them from the health department and everyone else accepted them.

I just feel like she was lying. She was quite upset that I turned in a white card and she was insistent it was a different color - but couldn't remember which. And the card she gave me was white.

Also, I feel if she wanted me to submit the right form she should know which form that she needs. It would like IRS telling you need a form but you go figure it out and they don't supply them.

My dd took the form back and she had no problems. Just feel harassed.
