I have to take the left side off duty - do I pump? Nipple shield with 2.5yo?

My son is just over 2.5, so this is no where near the critical issue it was when I had severe cracking on the right and had to pump one side every few hours for my newborn.

I think - but I'm not sure - that my left nipple got intensely bruised yesterday am when we were cuddling and he was thrashing around and knelt right on my (droopy, saggy, oh well) nipple for a moment. I recall it pretty clearly because I actually had to shove him off, and it hurt a lot at the time. A little bit last night, and today a LOT of pain nursing on that side. I kept coaching him about no teeth, gentle nursing, etc, but I see he's really doing the same on both sides and it's only the left that makes me want to cry with pain.

I just nursed him before stories with Dada, and it was so bad that I told him that this side has a little owie and will have to take a break for a day or so.

I'm not expecting to get engorged, but I'm a pretty good milk-maker...

Do I just leave it all alone and watch for discomfort and then express? Do I pump?

And, any tips on healing this?
It's major pain, and from my issues when he was tiny, I think I'm a pretty good judge of nipple pain...

I just don't know how to handle breast health when the "baby" isn't a baby anymore, and the milk production is still med - high for this stage, and weaning is definitely in my sights (though not his.)

FYI - now that we're home from vacation (a nursathon) he's mostly just morning, nap, evening, and bedtime, plus recovery nurses after big tantrums or falls. So I'm totally fine with supply dropping, if that happens.

THANK YOU!

My update - had some discomfort today so I expressed by hand. I checked up on the physical experience of weaning at kellymom and decided to start a medium dose of sage tincture since the nipple really doesn't seem better.

Now I'm wondering if a toddler/ preschooler can work with a nipple shield... gonna edit the title to see if that gets any experienced mamas to join in.

I have actually wondered that about the nipple shield too. I don't know if I have thrush back (had it almost non stop from 9-16 months). I don't think it looks like it but other than her teeth - which dig in more than I ever thought humanly possible - I don't know what else could be causing such pain. The left is much worse and I have considered dropping that side all together or even reintroducing the nipple shield (we used it for the first 5 months). She is 22 months and neither of us is anywhere near ready to wean but sometimes with this pain lately I don't know how much longer I can go. It seems worse around ovualtion but I don't know what to do with that information either! I'll be watching this to see if you have any information!

Update from OP - I see I started the thread on 10/6, and here it is 10/21 - only 2 days ago my pain seemed to be mostly gone. I've been very happy with the sage tincture for decreasing production, and thankfully my poor overworked right nipple was able to hold out solo. We're easing the left back into circulation, just a few minutes a day so far and so far, so good. I'm amazed it took sooooo long to heal. I really don't think it was a blocked duct - the pain was so specifically in the nipple, but it didn't stay in just one spot... it kind of migrated from 12 o clock to 6 o clock so I think it was a bruising and the extra fluid descended, if you see what I mean.

Anyway. Better now. Never tried the nipple shield. Just wanted to say that for posterity, I guess.

I had a really bad bite (a couple of times ) that took the boob out of commission. I tried the nipple shield, and when he was about 12ish months, it frustrated him and he wouldn't use it. Now (at 18m) he thinks it's a toy and tries to chomp on it. Like you see kids do with a sippy or paci, where they kinda bounce their teeth on it because it's plastic. Well, needless to say, that hurts more than nursing with a bum nipple.

The only thing I could do was go easy on that side, and just struggle through the (red-hot-flames-of-hell-searing) pain when I would get full on that side. I even tried pumping but that got me nowhere and was a fiasco.
