TMI post - hemmoriods and pushing?

I had bad hemorrhoids last pregnancy due to zofran constipation. One wouldn't stop bleeding so I went to the ER and they put stuff on it (surgi foam, I think) to stop it bleeding and it healed. I pushed for 3 hours for dd #1, and other than I assume normal issues, I didn't have problems, Although I had a c-section, so I am not sure how much that prevented further problems with hemorrhoids or not

This time I had to go to the ER again because of zofran constipation again- and they had to do surgery to remove a clot in a 'riod. But of course, it doesn't seem to be healing as well as the natural rupture before. It's been a month and I still have some pain in that specific area and sometimes it bleeds even though I am not constipated anymore.

So I am worried about pushing if it will be a major issue and cause it to rupture or bleed again. I am oddly very nervous about this because I really want to vbac and I don't want this to be a stupid reason to need a repeat c-section, but I have been on the end of a ruptured hemorrhoid and apparently you can lose a lot of blood from them! Should I bring this up to the midwife to ask? Do you think it will heal by then? I am 27 weeks now, I imagine it might get worse as I get bigger? i have read that bad hemorrhoids can be a reason for a c-section - is that true?

I generally have rhoids... just, I have IBS... it makes me constipated and thus I get some really seriously bad ones.

However, pushing in labor has never caused any problems with mine. I think it helps that the pushing is not focused on that area so much but higher up.
