New Mexico UC Birth Certificate Guidelines

Hey Mamas! I had a successful UC here in the state of New Mexico 4 months ago and had a really hard time with the vital records office in the beginning to get my baby's birth certificate.

I want to make it easy on any other mamas who are planning or already had a UC in New Mexico and need to get a birth certificate.

So here is what needs to be done to get a birth certificate in NM if you UC'd:

Provide the following documents for the Vital Statitistics Office (preferably in person):

Proof of Baby (This can be a doctors visit, midwife visit, christening certificate, etc.) (The documents must be the ORIGINAL!)
Proof of Pregnancy (The documents must be ORIGINAL!)
Proof of Marriage (Copy of Marriage Certificate, only required if married before said pregnancy)
Proof of Birth (Notarized statements from all parties present at the birth stating their full name, the date, time, and location of birth, the name of child born, and verifying the fact that they were present for the birth.)
Proof of Residency (Utility bills for the month prior to and the month of the birth)

Along with a birth certificate request and $10 check. Once these are turned in and approved, a live birth registration form will be granted. Upon filling this out the documents will be processed and a birth certificate granted!!

Ditto had the same experience w/ all my NM UC babies. Although, w/ a few I refused to submit a proof of pregnancy and they just had to get over it. I never brought papers in person, always mailed them.
