Sudden unilateral nipple pain

I haven't hurt this bad since he was a tongue-tied newborn. It's only on the right. It started 2 days ago and it literally takes my breath away when he starts nursing. It's right at the latch and then for the first couple of minutes. My nipple looks okay, maybe it's a little redder than the other one. When I rub over it, it doesn't hurt on the surface. It's more of an inside pain, if that makes sense. I work all day and pump several times, even pumping is extremely uncomfortable.
Any ideas?

I had this about 3months PP with DD2 (so 6 weeks ago or so). My right nipple only, and i NEVER had pain when she was new so it was the first time. After 2 days of sore nipple i got a red streak and then did everything i could to avoid mastitis (and managed it). I think i basically had a little block or constriction in the ducts somewhere behind the nipple. Does changing position help? Ask him to lay the other way or i don't know, stand on his head (they get pretty acrobatic at times, right?)? Someone who knows more will hopefully come soon and help more!

I wonder if it could be a duct problem. I will start to feel okay after he latches and then I let down and get a searing pain in the nipple that leaves me with fists and jaw clenched.

I can't believe you're stuck with just me for advice! Have you tried massaging back from the nipple, to see if you can find a painful spot or a lump or anything? I think for me it hurt so bad because it was right in the middle, not near any surface, so it was hard to locate the precise point of pain and hard to figure out what was going on.

I get this a lot here. I don't post a lot so I don't get many people willing to respond, I guess. I appreciate your help and taking the time to answer.
I think it's thrush. I looked at the boy's mouth last night and he has it. I started Diflucan for us both (we've had this several times, but it's never hurt like this) last night and I already can feel a difference today.

Sounds like it could be a clogged duct, but not sure about the nipple pain? The several times I had pain from a clogged duct, massaging seemed to help relieve the pain and I think applying heat could also help. I know this is not easy when you're working all day, but maybe you could take breaks to do this?
