Yeast??? Not peeing....12 hours?!

I posted a couple of days ago about possible tearing that is causing my son to hold his pee for extended periods of time.

Sun- two catheters (definite irritation) - diagnosis: UTI

Mon - one catheter since the bladder scan showed 200mL of urine in his bladder and he refused to pee (nurse was excellent - used numbing gel and he actually started peeing after the numbing gel took effect and she had just started to insert the tube

Wed 6:30am - hasn't peed since yesterday around 6pm.

We are pretty sure he is now suffering from a yeast infection, so we're going to try to get into our naturopath and wholistic allergist to see what they can do.

Should I be bringing him back to the ER to drain the bladder now? DH says it's too traumatic, that he'll eventually be forced to let it out (we were thinking that would happen as he slept). I am concerned about his kidneys if urine gets too backed up.

Any help appreciated!!!

I am feeling so sick to my stomach right now...I had to take Caelib (2.5 years) into the ER yesterday since he just wasn't kicking a week-long high fever and was irritable. He vomited and was really out of it. While there, they decided it might be a UTI - you know what's coming next....catheter...first nurse, very gentle...I told her his is intact and she said no problem - it just makes it harder, but don't worry - I won't retract. I was happy about that. She tried and there was just nothing in his bladder.

Later on (3 hours later) another nurse decided she would try. I explained about the foreskin and she just gave me the look. She did retract even though I was yelling to stop! Caelib was howling and moving all over which was just making it worse...the other nurse (first nurse) was even saying that was far enough and not to go any further.

Sample showed evidence of a UTI. He has taken two doses of the antibiotics, but is holding his penis and even saying his penis hurts. The foreskin is covering his penis like normal and I can't see any redness or swelling, but he is just in so much pain every hour or so. He wants his diaper off and then on. He keeps wiping himself with a cool cloth. I tried to get him in a bath but he sprung out and said it hurt!

Could it just be the UTI? the tenderness from two catheters? something about the retraction??

This has been the most traumatic thing ever - for both of us!

Caths can cause infection and since he already has one I would personally not have one done. Have you tried letting him sit in the bath and urinate or go in a cup? Anything to lesson the sting.

Also if the cath isnt done right it can cause damage as you know and that will add a infection of the foreskin to the UTI. I wouldnt risk it personally.

Not peeing is serious. If you can't get him to pee, then yes you need to have him cath'd yet again, but I suspect you are getting into a viscous cycle here.

A few things to try before going to ER. Putting a soothing barrier cream on the tip (we used Burt's Bees.) Give him a comfortably warm cup of water to submerge the tip of his penis and pee in (make it a measuring cup so you can be sure he peed even if it isn't very yellow.) I've heard peppermint oil can make one pee, but I'm not sure how to use it.

Don't spend too long on any of these without results though. It's already been a long time without peeing, so if these don't work right away, take him to the ER.

The yeast infection may actually be an allergic reaction to the numbing cream, so I'm not sure they should try it again. If they can get a dermatologist to look at it in the ER, s/he may be able to tell if it is an allergic rash and decide if more numbing cream is a good or bad idea.


ETA: if you end up back at the ER for a cath, ask for a NICU nurse or a nurse who went to school in a non-circ'ing country (nurses trained in Ireland are common around here.) They are less likely to try to retract and damage tve foreskin while cath'ing.

Thanks for your replies. I did end up bringing him in since it was going on 14 hours and no pee! I explained my concerns about doing another catheter, but they told me the risks of reflux into the kidneys and kidney failure, etc. - worst case scenario. When the dr left to get the nurse to get the cath kit, I told Caelib what was going to happen. Magically when the nurse walked in, Caelib was crying, but kept saying, "I went! I went!" Sure enough he had....so cath averted!

Niastatin/HC cream seems to have helped since he was able to pee without crying around 1:15pm. We have doubled his probiotics and we're off to see our wholistic allergist. I did try the bath, but he was freaking out so much about the water hurting his penis, I just couldn't force him. Do you just do straight water or epsom salts or something??
