Birth certificate question for AZ

it has been over three years (close to 4 when baby arrives) since we had our last (and first) UC. the last one was in AZ also. dh and do not remember what we had to do to get the certificate. i looked over the website for vital records, it doesn't sound hard, but i can't find the form on line you have to fill out (the certificate of live birth).

anyone do it recently in AZ that can give me pointers. don't know if things have changed in the last 3 1/2 years.

kitty: i went to the state website, but i can't find the one form. i am wondering if you either use the birth/still birth form or you just get the form when you go down town. it looks like you can mail the whole mess in if you get it to them within 7 days of birth.

still if you find anything, post the link. i would love to see it. thank you
