It's a CYA situation. The first checked statement verifies that they told you (general you) to get vaccines, so it is checked already. The second checked statement verifies that they told you that you could have bad things happen if you didn't follow their advice, so it is also checked. The third releases them from liability. They can't release themselves, it has to come from the patient/legal guardian.
