Bad mommy form?

Hi mommies,

 

       DD and I live in maryland, she's up for her 4 month check up this coming tuesday. I left her first ped for several reasons, but one being that she felt strongly about me needing DD vax'ed soon. So I found a new ped who says she'll just 'talk' to me about it each visit, but won't push me. She did however, mention that I will need to sign some sort of form. After watching the forums for awhile, It seems as though theres the 'bad mommy' form and just a regular 'I choose not to vax' form. Whats the difference? How can I spot the difference when i'm asked to sign it on tuesday? 

 

       Also, does anyone know where I can find the form to get exempt from being put on the MD vaccine data base??? I saw it once somewhere, but I cant seem to find it! Thanks for any help!!! joy.gif

My kids are 8 and 11 and I have never been asked to sign any form stating I am declining vaccinations for my children. Personally I would refuse to sign anything stating I am putting them at risk by not vaccinating.There is absolutely no need for such a thing.Doctors just note in the medical records: NO vaccinations.

Yeah, I'm hoping to take the form home with me to sign instead of signing it there on-spot. I just don't know what the difference is between the bad mommy form & one just stating that i'm declining to vax. I'm not sure if that's our data base or not, it seems like it is - but for some reason I thought I saw another site. Thanks for your help!!!

Anything that says that you understand the risks involved with not vaccinating, or you acknowledge the dangers associated with not vaccinating, etc.  Anything that could be construed later (although to my knowledge this has never been done) by child welfare services to be you putting your child in danger by not vaccinating.

 

I would feel comfortable signing a form that says 1) you are choosing not to vaccinate and 2) you have had an opportunity to ask any questions that you may have of your HCP regarding the decision, etc.

I would cross out this, or add "or vaccines"
