New to VA tell me about religious exempt form

My family and I have just moved to VA from AL.  I had to go to the health dept to get our religious exepmtion form in AL for my daughter, and it was pretty easy to do. 

I have seen the form on the web titled "Commonwealth of Virginia Certificate of Religious Exemption."  Is that all I need to do- just fill it out in from of a notary?  What belongs in the line that calls for the Student ID number?  Then I would just take that to a place that I wanted to take her to for a MDO program??

Welcome to VA!  Just print the form out and fill out in front of the notary.  That is all I did when registering ds for public kindergarten.  I left the Student ID number blank.  I think the school assigns the students a number and will fill that in. 

I'm not sure about MDO programs though, we didn't take part in those.

Yep!  That's all you need.  I have had DS in 2 preschools, now in 2nd grade public school and DD is now in preschool (different place than DS)--and I have used the form at all 4 schools without any problems.  At two of the preschools they did comment that I wasn't the only person who had turned in the form (it was said very nicely, almost as reassurance so I wouldn't feel awkward).  And these are basic mainstream preschools.  At the public school they just gave it a quick look, stuck in in his file and moved on without missing a beat.  I'm so relieved!

I am in the process of getting her enrolled in a program for 2 days/week.  When I mentioned we used cloth diapers (not PT'd yet- argh!) and had an exempion certificate, both the places I toured said it was no problem!!  I was so relived.  You hear such horror stories about day cares not accepting cloth diapers, or having problems getting the Religious Exemption in the first place in other states, it is such a relief to know that I don't have to deal with that stuff!

Where in NOVA are you?  Anywhere near Fredericksburg?
