Girl Scout Day Camp Health Form - Vax Issues. HELP!

Trying to figure out what to do here - if I should just sign or sumbit an exemption.

 

DD's day camp forms have the following "I certify that my child’s immunizations are current as required by Texas Public Schools." She has an exemption with the state - how would you do this on the forms?

 

Am I correct in thinking just sign it, and note in the health issues a religious exemption to all vaxs? Or should I cross this line out and write state exemption under it?

I don't think it much matters either way you do it as long as you indicate on the form that she has an exemption filed with the state.

 

I suppose if it were me, Id sign the form and make a note under that line or in the notes section about the exemption.

When I had to fill out vax forms for preschool,daycamp,and all I either crossed it out with a N/A or wrote legally exempted on the forms.

If you have a valid exemption on file with the school, your child's vaccines are current as required by the state school system. I would sign the form and be done with it.
