first meeting with midwife for VBA2C ?'s to ask?

I'm meeting with a midwife next friday to meet for the first time and ask ?'s. She has an independent practice http://lunamidwifery.org/# I would love suggestions for ?'s to ask. I'm going to make a list so I don't forget anything. This will be a home birth. Both my previous pregnancies ended in c-sections-the first emergency and second scheduled so this is entirely new territory for me. thanks.

I found several lists online that I've merged and added to and taken some away here is my list of ?'s thus far:

Prenatal Care

1. How many births have you attended as the primary caregiver? How many breech? How many VBAC/VBAMC? Outcomes/risks?
2. How many births are you attending now? Do you have a maximum, what if  2 at once? Have you missed a birth?
3.  Do you provide supplies or literature to help us prepare?
4. How often will I see you? What do your checkups consist of? How often do vaginal exams? Ultrasounds?
5. What are your guidelines concerning weight gain, nutrition, prenatal vitamins, and exercise? What are your standards for pre-eclampsia? RH-
6. Do you require that I take a childbirth education class? Thoughts on hypobaby?
7. Who takes over for you if you go on vacation or get sick?
8. What if I go over Due Date?
9. Do you ever strip membranes? w/consent?

Hospital and Obstetrician

1. How do you handle emergencies? Under what circumstances would you transfer?
2. Are you certified in CPR? What complications have you seen and handled? How would you handle complications such as hemorrhage, cord around the babys neck, baby not breathing spontaneously?
3. What are complications rates? How many women end up with an obstetrician? How many for pain, exhaustion and failure to progress, how many for medical complications?
4. Have you ever dealt with a major bleed?
5. Have you ever inserted an IV after a major bleed has begun? What would you do in such an instance if you could not get it in?
6. What are you death rates? Include stillbirth before labor, stillbirths during labor, infant deaths, and maternal.
7. What are your permanent injury rates? Include moms and babies?
8. If my baby needed a c-section to save her life how much longer would that take than if I had an obstetrician?
9. Would you stay with me in the hospital? Where would I go? my baby go?
10. Do you require that I see a physician during my pregnancy even if everything is all right?

General Labor

1. What kind of equipment do you bring to a birth?
2. How do you monitor during labor?
3. Can I eat, drink do whatever I want during labor?
4. Are you permitted to administer any drugs during labor?
5. How many people are allowed to be around?
6. When should I call you after my labor begins?
7. How long if it goes slow?
8. How much time do you allow for the delivery of the placenta? Do you ever manually help?

Financial Matters

1. What are your fees and what do they include?
2. What payment arrangements do you make?

The Baby

1. Have you ever had to resuscitate a baby? Do you suction airways?
2. How do you examine the baby after birth?
3. Do you give eye drops or shots to the baby?
4. Do you have a pediatrician you work with or recommend?
5. Do you provide or know of anyone who will help new mothers after birth?
6. Position on co-sleeping?

There were some ?s about her training, beliefs and such that are answered from her website so I left those out. Any other ?'s? Should I pick out my top concerns lets say 10 questions and just ask those?
