GERD - Med Changes

I have a 7 1/2 month hold who was diagnosed with moderate to severe GERD back at the ed of September.  We were discharged from the hospital with Prevacid (name bradn) 2 x a day.  We take a 15 mg solutabe, dissolve it in 5 ml water, then draw in 1.5 ml and give that to him.

 

We had a baby check up the other day, and I mentioned we only had 1 refill left that I was getting ready to call in to pick up while  at my other son's speech therapy on Thursday.  I got a text 2 days later from Sams that they had a prescription ready.  I went to pick it up, figuring I would use it instead of the one last refill.

 

I didn't read through stuff, just signed for it, and then opened it and looked at it, since it felt off.  It was different than we had previously had.

 

It was the generic Lansoprazole.  It was also written for 1 time a day, not 2.

 

I put a call in to the ped, on Friday and have not heard anything back.

 

Problem is that after 2 days of 1 dose a day, he is not wanting to nurse.  He is back to turning and contorting his body all over the place.

 

Is it from the dossage decrease or change to the generic or something else?

Sorry he's having problems again! 

 

Couple questions:  1. Is the mg the same per solutab for the generic, meaning, 15mg solutab still? 

 

2.And, did your doc say anything about lowering his dosage?

 

If the answer to the first one is yes, and the second no, then maybe it was just an oversight, hopefully when he/she calls back can clarify.

 

If the generic new scrip is actually a 30mg, maybe they were thinking, instead of 2 doses at 15mg, instead 1 at 30mg.  But, generally speaking, and in my own personal experience with DD, having two doses a day is much better and continuous control than one. 

 

Regarding the generic, you could call a pharmacist and ask if they can provide an ingredient list for both to compare.  But, if he was previously getting it twice a day and it was well controlled, i would think it's more likely to be the dosage issue.  it's a lot of trial and error with these meds and our babes.

Dosage decrease I'm certain. The doctor messed up (or pharmacy..have you talked with them...here they will contact the doctor directly with things like this). I think the actual dosing guidelines call for three doses at his day in fact. The issue is that young babies metabolize this faster. I think one dose a day is worse than none as you get rebound acid and I'm sure that's what's happening here.

MarciKids website will have dosing information (including frequency and weight). Check that as I bet he was underdosed from the get go. I'd dose him as regular until the pediatrician can fix this problem.

Actually he has done fairly well on 2 x a day.  When we were in the hospital for it, it was horrible at night, he was back to fussy and screaming.  Even the nurses noticed the difference between morning and night.  That is why they went to 2 times a day.

 

Yea, I saw the rec was for 3x a day.  But again, the 2 seems to have it under control (he is also more towards the upper end of dossage for his age), even his visible spit up has signifcantly decreased.

 

I will call in to the Peds again in the morning.
