Have you heard of this relating to GBS disease?

My DS and I dealt with some sort of infection during labor.  I was GBS+, and did not have antibiotics started until 2 hours after AROM when we transferred to the hospital.  In all we had antibiotics for about 3 1/2 hours before DS was born.  About 3 hours before birth he began to have deep decels in his heart rate that did not return to baseline inbetween, and he was in respiratory distress at birth.  After the birth I spiked a high fever.  We were in the hospital for another 2 days with both of us on IV antibiotics.  DS's vitals and respirations became normal about 24 hours after birth during which time he was on antibiotics, and basically just being held skin to skin.  After we got him clamed down after birth, and a little time on the cpap, they felt he was "good enough" to room in with us.  (Love our baby friendly hospital.)  His GBS culture came back negative 48 hours later.

 

One of the midwives attending said her and the OB felt the decels, DS's subsequent respiratory distress at birth, and my high fever, were due to chorioamnionitis from GBS.  She said that when antibiotics are began during labor the GBS culture on baby is not always to be believed if it does come back negative.  In other words, it could be a false negative, and they look more at the circumstances surrounding the birth to diagnose what happened.  I have never heard of that, and just wondered if anyone else had.  My midwife has years and years of experience, so I trust her.  The OB I had never met, but is respected in our town, and also has many, many years of experience.

GBS should not have caused your fever.  It is likely that you did indeed have chorioamnionitis, but not from GBS.  Did they culture your placenta?  That would tell you exactly what bacteria caused the infection.

Why would GBS have not caused my fever when fever in the mother is a risk factor for a baby developing GBS disease?  Just wondering?  My fever spiked immediately after he delivered.  The midwife and OB suspect a uterine infection developed in labor.

 

They did not culture my placenta.  I wish they had.

Oh my gosh - I'm an idiot.  I totally missed where you said you were GBS+.  Duh.  I'm so sorry.  Yes - then the chorio certainly could have been caused by GBS. 

 

As far as the false negative on your son's culture, I'm sure it's possible.  We usually try to draw blood cultures before starting any antibiotics when we suspect infection for that reason.

No prob.  They did draw before they started his antibiotics, though I had had them during labor, so I guess that means he would have had antibiotics in his system then at the time of the draw?  Anyway, it does not matter too much as he is fine and I am fine, but I was just curious.  Thanks for answering.

What your midwife stated is a true statement about the timing of antibiotics and blood cultures in general: if the antibiotics are started before or during a culture, the results may come back negative even thought the patient actually has the infection. This is true for GBS and any other infection. This is why the clinical picture is so important and requires careful assessment.

Thanks for answering.  We had a complicated birth--what was intended to be a home birth!  Anyway, I'm sort of in the process of making sense of everything for myself.  All our midwives and the back-up OB, who ended up getting called in to help in our birth, have been great in helping us to understand all that happened, but once in a while another question arises.  So thank you all for helping me understand this part of our birth better.  I appreciate it!  :)

I'm very sorry that you had to go through such a difficult and unexpected experience, and am sure you will continue to have questions (I always do, and about every little thing -- I tend to obsess about all the unknowns myself). Ultimately, it is so good to hear that your new baby is now doing well! Congratulations!
