Nebraska state forms

I'm WAY new to this, so these might be dumb questions - haven't started hs'ing yet, but making plans.  I was looking at my state's required forms to file.  They ask for the curriculum being taught for each required subject area.  I'm kind of piece-mealing it together, at this time.  Montessori/Shiller Math, Lisa Nolan's Montessori Homeschool and/or CM for other areas.  I think these philosophies overlap in many ways.  Is it okay to present a hodge-podge like that to the state, though?

 

Also, ds works above grade level in some areas and is on grade level in other areas.  One section on the forms asks which grade level will be taught for each of the major subject areas.  Do I just put his grade level (1 or 2, depending on when we start), or do I put the levels he's actually working at for each subject, thus determining which part of that curriculum I am using.  Does that make sense?  I know I'm probably over-thinking this.  If they had made it a straight-forward, all-inclusive question, encompassing all subject areas for each student, then I wouldn't be confused.

I think it would be very helpful if you'd put the name of your state in your subject line - you'd get input from people who have a sense of what's expected there. Each state is very different from the next, so you need to talk with people who have some experience there. If you have local support groups, they'd be good to contact - and you can look online to see if there are email groups on a state level that discuss homeschooling there. In CA, for instance, you'd get a lot of good and detailed information if you posted this question in our state homeschooling organization's email list that has around 3,000 members. Thank you for the feedback - makes sense!  It was late when I posted, and I wasn't thinking.  I am starting to get in contact with local groups, as well, but I figured I'd see if anyone here could help guide me.
