Splinter under the nail

My eight month old daughter got a splinter underneath her nail.  It goes all the way to the base.  I can't get it out, it's too deep.  If I take her to the doctor, they'll want us to do a tetanus shot, antibiotics, and do God only knows what to the nail. 

 

Any advice?

You can reject the tetanus shot and take the prescription for antibiotics and not fill it (unless it does get infected).

 

If you wish to try to treat it at home, you can try an epsom salt soak to soften the area, but I do not know how well that will work for a wiggly 8 month old. I certainly can't picture my 9 month old staying still long enough for a soak to do the trick unless i gave her an entire bath in epsom salts.
