You've had some good advice. Definitely seek independent legal advice about potential issues with granting an easement - both now and in the future. Local realtors may be able to give you an opinion on the value of the easement - not only how much to charge, but also any negative impact on the value of your property if you decide to sell it. I'd include your legal costs, property value assessments etc. in the final price to the doctor's office - you shouldn't be out any money since this is entirely for their benefit.

I also wanted to say that we put in our easement that if the school (to whom we granted the easement) ever ceases to be a school, then the easement is null and void and will have to be re-negotiated with the next party.  (So if the school is ever replaced by a Walmart, we can charge more!)
