JK registration in Ontario - who can sign my vaccine exemption form???

I need to register my son for kindergarten this week and he has not been vaccinated which is a requirement here in Ontario.  I printed the exemption form off the internet

But, it says "sworn or solemnly affirmed before me....by a Commissioner etc. 

We have an appointment tomorrow with our family doctor.  Does he qualify to sign this????

You need a notary public. You go to them and you sign the form in front of them, I think you have to bring some photo ID, to verify who you are. I only ever used a notary once when we had to get our marriage certificate and that was in a different province. You should be able to find one in the phone book.

Our city hall has a person that can act as a commissioner.  You can try calling and asking at yours.  I think the fee is fairly low.  I got our form signed by a friend who is a lawyer.

ETA: I just Googled and your city website has a commissioner listed and it is $32.  I'm just a bit north of you and figured it would be similar.

Town Hall - mine was 10$.

You will then probably have to mail a copy to your Health board and you might want to give a copy to the school.  Keep a copy for yourself - the health unit lost mine

I see that you are in Burlington, I just moved here and am hoping for a good pediatrician that is open to working with me on my preferred vaccination schedule... Is you dr cooperative? Any recos in the area?

Just take the form to the office on Bronte Road north of the QEW (where the Halton Regional Police headquarters is, somewhere in there) and they will do it for you. http://www.halton.ca/cms/one.aspx?objectId=9090
