Domperidone vs Reglan?

My baby is now 6 weeks old and we discovered my low milk supply at 1 week so I've been pumping for 5 weeks now.  I started domperidone at about 2 weeks.  Luckily, I have a doctor who is bf'ing-friendly and not afraid to prescribe domperidone.  I haven't seen much of an increase since I started the domperidone.

 

At first, I was pumping about 1oz per session.  It finally crept up to about 2, 2.5, or one time 3 last week.  Now, for about a week, it's been back down to 1oz or 1.5 at the most.  I don't know what I'm doing differently except sometimes I'll sleep for 4.5 or 5 hours at night (I was EXHAUSTED, even with my one 4-hour stretch at night that the LC recommended).  The only other thing is that I'm pumping 20 minutes now as opposed to the 35-40 I was doing before.  The LC was suprised I was pumping that long because I also have thrush and staph so pumping was extremely painful.  She said the goal is really to pump for about 20 minutes every three hours.

 

I've tried pumping every 2-2.5 hours for the last 36 hours of so.  Still getting just an oz most of the time.  I'm on Motherlove Special Blend, I eat oatmeal, I drink tons of water, hand express after pumping, nurse occassionally to let the baby stimulate me and I'm on the highest dose of domperidone and have been for about 2 weeks.

 

The only thing left to try is Reglan.  Has anyone not had any increase on domperidone but on Reglan?  I am so frustrated that I'm going backwards.  My LC says by 7-8 weeks, your milk supply peaks and then I'll know how to move forward.  I am so sad that I thought it was going up and now it's not.  I feel like I only have 2 weeks to see what my body will do and it's not looking good.  My son eats about 24 oz a day and I was producing about 12 but now I'm back in the 6-9 oz range.

I was on Reglan first, then switched to Domperidone. Reglan did work better in terms of milk production, but it made me so horribly depressed that I couldn't stay on it. I'm doing okay on Domperidone, but just barely, and I can't get ahead at all. My doctor asked if I wanted to switch back, but my baby is doing okay so I'm sticking it out even though it means I'm nursing pretty much non-stop around the clock.

 

It's probably worth noting that I have seasonal depression in the first place, and got hit really hard by postpartum depression as well, so adding Reglan on top of that was just a nightmare. From everything I've read, my reaction was on the severe side - it sounds like most women tolerate it better than I did.

 

I'm so sorry you're having supply problems too. I know how hard and frustrating it is, especially after having to go through IVF.

I am on DOM and you wrote me on another post. I was told it can take up to 6 weeks to see the effects of it in some people. I too have not seen a big increase. I am on my fourth week but just 1 week not even of 160mg/day. So I am going to wait and see.

Oh, I'm glad you started this thread because I have been wondering the same thing! I have been on the highest possible dose of dom for 8 weeks and it hasn't done anything, so I've been wondering whether it is worthwhile to try Reglan or not. I'm so sorry you're in this situation...I know how it feels to desperately want to be able to produce a full supply when it is just not happening no matter what you do.

To the OP - how much Dom are you taking? You can take up to 160 mg/day if you need to. Sometimes doctors only prescribe 20-30 mg, and that's not enough for lots of ppl.

I am taking 160mg/day and have been for about a month.  I started at 40mg/day and then after a week, went to 90mg/day for a week.

Yes, I saw your response and have kept in mind that it might take that long.  I've been on the 160mg/day for about 3 or 4 weeks but been on domperidone for 6 weeks total.

Very curious to know others experiences.  Taking Dom last time around (unsupervised by a Dr) made me nervous.  I never tried Reglan because I was worried about the side effects, but I might with baby #2...

Update:  so far, I haven't had any side effects on the Reglan.  I've been on it for about 5 days.  My supply hasn't really increased much.  I'm averaging about 1.5 oz per session so about the same but at least I'm never pumping less than an oz like I had been before I started taking it.  I wish I knew why I had been up to 2.5-3oz for a few days on the domperidone.  That didn't last though.  DS is eating 4oz every three hours now so I'm trying to stay positive but I'm not making nearly enough for him.
