Best remedy for thrush?

Me and my baby have thrush right now.  I took him to the doctor (convienent care though, as my pediatrician couldn't get him in quick enough) and she prescribed the nystatin suspension to be squirted on each cheek 4x a day for 14 days.  She told me to use Lotrimen on myself.  When I went to the store to get it - I found it with the foot fungus creams and didn't want to use it on my nipples.

So what should I be using on myself?  I have read the nystatin suspension is not as effective as miconazole oral gel.  Should I switch my baby to that?  If I use a miconazole on myself, will that help baby more?

Do I need to do something special with my babie's toys?  He puts everything in his mouth right now.  The doctor said to just wash everything with mild soapy water - but will that kill the yeast?  And should I do that now, or wait until the infection is gone so he doesn't re-contaminate everything.

I was told Fluconazole orally (the Newman protocol) and either gentian violet or grapefruit seed extract on the nipples.  GSE is easier because it doesn't stain.  Both for the mother.  No one could give me a good answer on what treatments work for baby.  We were also told to boil everything that comes in contact with the baby's mouth for 20 mins every 24 hours!  But things that couldn't be boiled, we could rinse in equal parts vinegar and water.  Don't know why we had to boil the other stuff but that's what we were told by the doctor and LC.

The protocol most often suggested in the UK is here: http://www.breastfeedingnetwork.org.uk/pdfs/BfN_Thrush_leaflet_Feb_2009.pdf
