Who accompanied your midwife?

For those of you who have HBACed, who came to your home with the midwife? An assistant or nurse, no one? If a MW doesn't have an assistant or back up would you be concerned?

When is the risk of UR the greatest? I believe I've it's during labor - not pushing. If that's the case wouldn't a home birth MW want to arrive to a birth sooner...rather than later like tor the last hour of pushing?

My midwife will bring her student, who has been a doula for 10 years and also apprenticed with one of the most experienced midwives in our area (and that is my midwife's backup, that experienced midwife)before coming under my midwife. They want two at the time of birth in case baby and mama both need attention. If student can't make it, my midwife will call her backup to come in time for the birth.  I would be concerned if the plan was for only one midwife to be present at the time of birth and the immediate post partum hours.

 

I think at least for my midwife, she doesn't feel the need to come early because the risk is so small for UR, less than other birth problems, that it (and to me too) is placed in it's proper order of risk.

Would you be concerned if the hone birth midwife didn't have an assistant?

I just had a HBA4C. My primary MW usually has an assistant, but the assistant was out of town during the time I was due. We did end up having one of the local MW's present as an assistant instead. I think most MWs like having a second pair of eyes and hands during the labor and delivery.

 

I'm not really sure when the risk of UR is the greatest, but I do know that it can happen in women who don't even have uterine scarring. I think it probably stresses the scar tissue when you have the strongest contractions - well into the labor process. But a MW should want to be there early for a VBAC just so she can assess you early on and watch for danger signs.
