need help with kindergarten registration shot record / exemption

I'm asking this for a friend who needs help... for the kindergarten registration papers with the form to be filled out by the dr which includes the record of immunizations... is the dr required by law to fill it out?  Her child has had most of the shots on schedule except for one, and also not the 4-6yr boosters. What is recommended here?   Leaving the entire thing blank and attaching an exemption or filling out the past shot record? In our state we have only medical or religious exemption.  So I told her to leave it blank, put a big X over that part of the form and write in "exemption attached"  - is this good advice? She is worried the dr might have to fill it out by law.  I don't think thats true but I am double checking here to see what is recommended. Its been a while since I brushed up with this info and want to make sure I am giving her the right advice. I am sure lots of you have been in this position.

you gave the correct advice. If your friend is filing a religious exemption, you can leave that part blank. You doc could fill it out as well since one can file a religious exemption at anytime. You just can't get vaccines AFTER you file an exemption. There may be some benefit in filling out the record. If there is an outbreak in the school and her child has had certain vaccines, he/she may not be excluded. I know that just turning that blank part and attaching an exemption may raise less question however.
