I have insurance with an annual cap of $2,500 for infertility diagnosis AND treatment. I'm not yet at the 12 month mark of trying, but I suspect it's possible I'm not ovulating. I'd like to have the initial tests done (whatever they may be) to see if my hormone levels are normal, but I'd like to have those tests done as part of an annual physical...because insurance covers annual wellness labs and visits 100%. Are hormone levels a specific test that would only be reported when infertility is the reason, or are hormone levels reported in a normal annual workup? (I've never had a blood workup as far as I know, only annual PAPs.)

Has anyone had hormone levels checked at an annual physical?
