another birth cert question

I just got the application for a birth cert here in texas and am seeing that there is 3/4 of the paper that has nothing to do with  naming my child.  it's all public health stuff that won't go on the cert.  Am I required to answer all those questions?  can I just kindly opt out of all that info?  Thanks

Did you UP in addition to UCing?

I'm UPing and UCing and when I look at the reqs to get the birth certificate, I worried about the UPing part, as the site I looked at seemed to need confirmation from prenatal caregivers.

no I did not UP.  I have a mw who is doing my prenatal care but does not do homebirths.

I did mine in Pennsylvania. I answered those questions as best I could, using "n/a" whenever something just didn't have the answer required. 

I also did a UP. In order to get the birth certificate for a UP/UC, I had to have two notarized forms submitted with the paperwork that I filled out. Those forms asserted that these women would verify that I had been pregnant and that I had the baby on X date. Easy enough -- cost me $30 to have their letters notarized.

And that was that.
