3.5 yo potty issues pls help!

my dd is peeing and pooping in her clothes and all over the house, several times a day. i am at a total loss as to how to deal with this! here is a little back ground, and any thoughts or suggestions are welcomed. i have got to get to the bottom of this.

dd started taking off her diaper and peeing and pooping around the house several times a day at 1.5. she would also put on her clothes without the diaper and go in her clothes. when she began potty learning, she was excited at first to be wearing undies, and it stopped, but then she went right back to doing it again. just after she turned 3, when i was certain she was fully capable of using the potty all the time, because she would be able to do so for 1-2 weeks at a time, (of course, i am not referring to occasional accidents, which are common and to be expected) she started right back up again, peeing and pooping anywhere she happened to be, clothed or not. i tried going back to diapers. not as a punishment or in any sort of shaming way, but just to help her until she felt ready again for underwear. she got so upset as if i were punishing her, and screamed and fought me every time i put a diaper on her. so after about 2 days, we went back to undies and she went for a while without incident. but, she's back at it again. its now been going on, off and on, for the last 5 months. most recently, she's on an approximately 4-week stretch, going anywhere from 2-5 times a day. she'll just pee and then keep right on playing, not even telling me she went until i step in it or my older ds tells me. then sometimes, she will use the potty. there is no rhyme or reason to it. yesterday, i said i think we just need to start over with the potty learning, because you seem to have forgotten. she agreed, and we put her in diapers. all morning, she told me when she had to go and she went on the potty. she tried taking her diaper off a couple of times, but i told her it needed to stay on. sure enough, just a liitle while ago, she took off her diaper in her bedroom and peed on the carpet. the bathroom was literally steps away.

i am at my wits' end! i have no idea how to deal with this and am so sick and tired of doing pee and poop soaked laundry and cleaning it up from floors and furniture. i also have her help me clean up every time. it does not seem to have any effect.

My daughter did NOT like to interrupt her playtime to pee!  So I actually placed the potty immediately behind her wherever she played and pointed out that the potty was RIGHT THERE.  She went naked during this time, and I kept a careful eye out for the "potty dance".  It was not exactly easy, but I felt that it was easier than battling this strong-willed child.  What was different is that she seemed panicked when she had to poop and didn't have her diaper on.  I almost relented but I told her we were done with diapers and it turned out not to be a big deal (I felt pretty lucky, actually).  Eventually we moved the potty to an easy-to-reach location near the play area, and there it stayed (unfortunately) for the next 3 years because DD2 started potty training too.

     When the clothes were on and I'd see the potty dance I'd do a chant: GO!  GOGOGOGO!  GOGOGOGO!  She does the potty dance!  Gooooo potty!"  Somehow it helped her "make it" to the potty.  For DD2 I started a little cheer:  "She did it!  She did it! Yeah! Yeah!  She did it!  She did it!  She really, really did it!  She did it--who did it?--(name, name) did it!  She did it!  She did it!  WHEW!"  And, phew!  Isn't potty training more work than washing diapers?  I am still getting DD2, now 4 1/2, to use the potty so she won't have an accident.  And she will have one, too because she just won't stop playing!
