Applying for Unassisted Home Birth?

Anyone have some tips on how to go about apply for a birth certificate? 

 

I have most of the necessary items needed-

I have the "affidavit of home birth" form for unassisted births.  I'm assuming ANYONE can sign that- including my mother?  She'll be here for the birth caring for my other kids.

 

I need to get a Form Certificate of Live Birth.  Any idea on how to go about getting that?

 

The rest is pretty easy I think (maybe I'm over simplifying, but it seems simple enough)-

  • Evidence of pregnancy- Have a "proof of pregnancy" sheet from local health clinic.
  • Evidence of mother's presence in state before and after birth- Can give a copy of my electric bill since it's in my name.
  • Camera dated photos immediately before or after birth- No problem
  • Credit or check purchases signed by mother from businesses in the state both before and after birth- Okay, I go to Wal-Mart all the time so I can just get bank statements for that.
  • State ID for me. 

 

Now what about the PROCESS?  I just gather these items and mail them in?  Do I attach it to the birth certificate application (like the form I would use to request a copy of my birth certificate) or would I need to actually go down to the Vital Statistics office in person? 

 

Anyone willing to give me some insight or a step-by-step of what all I need to do?

Have you asked over in the UC forum?  They'd be the real BTDT experts.  :)

 

Otherwise, a local dept of health might be the first place to check for the forms.  I know some moms locally who UC and I seem to recall one of them telling me she got all her paperwork from the DoH.  But I don't know if that's just because our DoH is more laid back than some, or if that's the place you go.

Didn't even think of that, lol.  I usually just stick in the DDG's here.  Thanks!

I had my last baby unassisted and all of my correspondence was through the Office of Vital Statistics.  It might be dependent on the state, though.  I didn't have to go in (thank goodness, since it was across the state) but I talked to them and they told me exactly what they needed and how to fill out the forms.

Yeah, I'm hoping to avoid the trip as well since it's about 2hrs away.  Blech, lol.

Yeah, I would just call the Office of Vital Statistics if you haven't.  They sent me a homebirth packet.  For things that weren't clear (who to list as the attendant, what would count in each category) I just talked with them to get it clarified. I ended up sending in a copy of my initial bloodwork from my doctor's office as proof of pregnancy and a copy of the information sheets with her weight/height that I got at her doc's office when I took her for her PKU at 9 days for proof of birth. 

 

There used to be a thread in the UC forum about what the requirements were for each state, although I have no idea how to get to that now.
