Question about rupturing?

My hospital gives the rate for rupturing at 1 in 200.  And if continuously monitored they can 'save' 4 out of 5 of those putting the death rate at 1 in 1000 for every VBAC.  

 

Does anyone know at what stage rupturing happens?  0-3 cms? 4-6? 7-10?  

 

What raises the change of rupturing?  I know some (maybe all) methods of induction increase the chance.

 

I am mainly asking because I would love to stay home until active labour but wonder if I should go in earlier just to be safe.

If a rupture is going to happen it will typically happen in the begining of labor. The rupture rate after 1 prior cesarean is 0.5-0.9%.  That is less that a 1% risk.  You have higher risks for many other things in birth that people dont think twice about.

Pitocin and being induced hightens the risk of rupture as does having a scar that is not low/horizontal.

Cervidel and cytotec are MAJOR no no's for vbac. They raise the rupture rate a lot.

 

Personally for a VBAC I would stay at home as long as possible before going in because they will want to hook you up to moniters and put in an IV and all that torture. But its your choice and you have to be comfortable in what you decide.

I can't locate the source, but I believe there was a study (or a few?) that didn't find a difference in early rupture detection between continuous monitoring or monitoring at 15 minute intervals (by doppler) in stage 1, and more frequently (but not continuous) in stage 2.  Wish I could find that for you - sorry.  I'm also interested in  information on when ruptures are more likely to happen, or rather what %ages happen prior to labour, early labour, stage 1, stage 2.
