Maine exemption form

Any non vaxers in Maine - I am looking for a philosophical exemption form, especially if it is one where I do not have to write down my reasons!  I have found a few online but there are from specific schhol districts and I also do not like the wording.  We are in RSU #13.

Maine has no official form. This is what the law says. "The parent states in writing a sincere religious belief that is contrary to the immunization requirement of this subchapter or an opposition to the immunization for philosophical reasons." http://www.mainelegislature.org/legis/statutes/20-a/title20-Asec6355.html

This is what I would do.
