Yikes! College Student transfer to University 2011

I was a Mothering mom probably from the beginning of Mothering and now I need some advice. 

My son is 21 and is transferring from the local community college to a private university in Wisconsin and several vaccinations seem to be required. The junior college never asked about vaccinations, but now it seems we need to write a new letter. 

Here are my questions:

-If he is a legal adult, who writes the letter?

-Are there any new laws or practices I need to be concerned about? Especially in Wisconsin.

I believe WI has a philosophical exception, so you shouldn't have too much trouble.  I would think he would have would have to write it seeing he is a legal adult.
