Pertussis & Tdap requirement shots

I'll be entering the 12th grade (even though I'm already 18) and my school along with most of the districts around here are requiring it. I wasn't really aware of this until I checked my school site. I attend a public charter school. My niece actually had to show up to her school to receive her schedule instead of it coming in the mail like it usually does, so they could check if the students got the shot. 

Anyways, I never really had to deal with something like this, I do not want the shot. So can I request an exemption thing from my doctor? I don't know if my school will check. I'd like to know what I need to ask my doctor/show my school. 

I'm really not digging this, I hope my school doesn't give me bs over this. It's because of the California Assembly Bill 354, which I haven't heard of until now. Anyways, help?

No. The Tdap vaccine has its own exemption form: the Personal Beliefs Exemption form (CDPH 8261). But that form has some obnoxious wording on it designed to intimidate people into not taking the exemption, so you might prefer to write your own letter to the school. This is what I would write: 

"I hereby request exemption from the Tdap vaccine requirement for school entry because the vaccine is against my beliefs."

The law does not require you to use a form to get an exemption. The law allows you to write your own letter.
