Need help finding the NC religious exemption form, please!!

My daughter is starting k-3 on Thursday, and they are totally ok with a religious exemption, but said I just need to fill out the form and bring it in with all of her other paperwork. I have looked, but for some reason I can't find it, and I honestly don't even know if I"m looking for the right thing. Just hoping that maybe someone else could help me find the info I need. Thanks!!

There is no form for North Carolina. You need to submit your own written statement.

One of the best websites for info in our state and the one I had my letter for the hospital from! It's nice to be armed with the letters before they demand them! Good thing your pre-k is so agreeable! Alot of them are not! Hope this helps!
